ATHLETE PRIMARY INSURANCE

 

ATHLETIC INSURANCE INFORMATION


The Central Methodist University athletic department does not provide a secondary or primary athletic insurance policy.  There is a deductible assistant program that one may apply for through the Director of Athletic Training Services.  The assistance is to aid in athletes and their parents in meeting their annual or term deductibles.  The maximum benefit for this is up to $1000 per injury with a maximum of 2 injury claims for the year.  More information is available at http://www.cmueagles.com/injuryclaims
Every athlete must complete an insurance form ANNUALLY.

Have a clear digital picture or scan (tif, jpg, or png format) of your insurance card ready to submit with this form.  A pdf will not work.


All CMU athletes must have their own insurance policy.  You will not be allowed to participate in athletics until you have proof of insurance coverage. Central requires all student-athletes to be insured under a primary insurance policy.  College students can incur significant out-of-pocket expenses for non-athletic related illnesses or accidents. No student-athlete will be allowed to practice until they provide proof of primary insurance.   If you currently do not have insurance, check out Blue Cross Blue Shield or Dissinger-Reed Insurance  or e-health or UnitedHealthcare.  These are suggestions as to sites that have had insurance options for uninsured ATHLETES.  Make sure your primary insurance plan covers you in the state of Missouri and covers you for intercollegiate athletics participation.

  • Please be aware that many insurers drop primary coverage of dependents once they reach the age of 21.  This can often be avoided simply by informing the insurer that the dependent is a full-time student.
  • If you have a PPO or HMO, the athletic department suggests you switch your primary care provider (PCP) to a physician in the Fayette or Columbia area to make injury care faster and easier.  Most insurance companies allow you to switch the PCP back to your hometown for summer and upon graduation.  If you need suggestions for a physician, contact Head Athletic Trainer Paden Kleinhesselink (660-248-6978) or email, pkleinhe@centralmethodist.edu.
  • AN OUT OF STATE MEDICAID INSURANCE IS NOT ACCEPTABLE FOR OUR INSTITUTION.  PLEASE CONTACT YOUR LOCAL SOCIAL SERVICE OFFICE OR THE HOWARD COUNTY OFFICE OF SOCIAL SERVICES AT 660-248-3324 FOR INFORMATION ON QUALIFICATION AND HOW TO QUALIFY.

 

Primary Insurance 

  • Each student-athlete must provide proof of his/her primary health insurance coverage, which must remain active for the entire duration of the upcoming school year. This policy must cover accidents sustained during organized intercollegiate athletics.  If purchasing a new policy, it is strongly recommended that coverage for athletics is verified through the insurance company.  A copy of the insurance card must be provided for proof of coverage.  If a student-athlete’s medical insurance is not in compliance with the insurance requirements of Central Methodist University, then additional coverage provided by the University’s secondary insurance policy may not be available.

International Insurance

  • International Insurance is not different than domestic insurance.  Make sure when you are looking to purchase an International Travel Insurance that you disclose to the company:
    • That you are planning to participate in intercollegiate athletics at CMU
    • That this policy will cover you the ENTIRE time you plan on participating with our athletic teams.  Month to Month coverage is NOT permitted.  It needs to at least be for a semester.  Any charges that fall outside of this term or is seen as a “Pre Existing” condition will be the ATHLETE’s or PARTENT’S of the ATHLETE responsibility.
    • If you currently do not have insurance, check out Patriot Travel Medical Insurance or Pioneer Elite Travel Insurance or Seven Corners Travel Insurance.  These are suggestions as to sites that have had insurance options for uninsured ATHLETES

CMU’s Insurance Deductible Assistance Program (Also see Injury Claims)

  • Participation in intercollegiate athletics involves the inherent risk of injury. For this reason, it remains necessary for all student-athletes to maintain a personal insurance policy throughout the school year in case of an athletic-related accident. The NCAA or NAIA does not currently require member institutions to purchase insurance to cover the cost of athletic-related injuries. However, Central Methodist University provides financial assistance for injuries that occur as a result of intercollegiate participation.
  • Insurance deductible reimbursement benefits are limited to those injuries and illnesses received which, in the opinion of the Team Physician and Certified Athletic Trainer, are directly attributable to participation in intercollegiate athletics while a full-time student at Central Methodist University. Notification of Injury MUST have been reported to one of the Certified Athletic Trainers in a 24-hour time frame.
  • The current program at CMU will not provide assistance reimbursement coverage for the following:
    • Costs incurred from the care of injuries and illnesses that are not linked directly to a specific athletic accident or event.
    • Costs associated with injuries and illnesses incurred while participating in activities not directly associated with the student-athlete’s intercollegiate program.
    • Costs incurred due to the treatment of pre-existing conditions.
    • Costs incurred due to additional testing required as the result of issues or concerns raised during the pre-participation examinations.
    • Costs incurred due to the treatment of chronic injuries.
    • Costs incurred from the emergency care of medical conditions that are not directly attributable to participation in intercollegiate athletics (ie. appendectomy, hernia, etc…).
    • Costs incurred due to dental care not relating directly to an athletic incident.
    • Costs incurred due to injuries associated with fighting (regardless of the setting)
    • Costs incurred due to prescribed medications
  • All bills must be turned in or sent to the Director of Athletic Training Service, Paden Kleinhesselink, M.Ed, LAT, ATC, pkleinhe@centralmethodist.edu or fax 660-248-6381.  Proof of the first $250 payment, which needs to be a receipt from a paid vendor, AND ALL Explanations of Benefits (EOBs) from your Primary Insurance must be submitted within 6 months of the injury.  CMU will pay direct to a vendor OR reimburse up to $1000 to the related bill(s).
