ALL Central Methodist University student-athletes must have a complete folder on file with the Athletic Training Department and be cleared before athletic participation is permitted. Intercollegiate athletic competition includes any official team practices, voluntary workouts with coaches or other athletes, any team structured activities, and any use of the university’s weight room.
A completed folder will include the required documents below, as well as a copy of the student-athlete’s current health insurance information. The student-athlete will be required to complete the necessary medical information, including emergency contact and insurance carrier information, through the Blue Ocean Portal system (The NExTT Program). All information is subject to review by the Central Methodist University Sports Medicine Team prior to any type of athletic participation.
We will not be accepting outside physicals, and all physicals will be conducted on campus.
Each athlete will be required to have the following 3 documents filled out and turned in to the AT Department prior to returning to campus:
- Athletic Pre-Participation Physical Evaluation-History Form
- Verification of Current Medical Coverage Form
- Copy of Insurance Card (Front and Back)

- Champions of Character and Student Handbook Form (Vivature/BlueOcean program in Forms Section)
